Questionnaire on Growth Mindset, Self-Regulation and Enjoyment

I have my first classes tomorrow in the new School and I wanted to get a general sense of student mind set and their self-regulation skills before we start moving on with the Mathematics. Can anyone think of any amendments or additions they would make to this questionnaire?

Update: The only thing I did to the questionnaire was to add one last question – “What do you hope to get out of Mathematics this year?” I’m interested to see some of the responses.
